Weight Loss for the Elderly: Why Medical Guidance Is Essential

Is medicated weight loss just for the younger generation? Nope!

When we look at statistics around medicated weight loss, it has been noted that the largest demographic who are undertaking this treatment is aged between 50 to 64, according to Forbes. Although the percentage of adults aged between 18-39 who have been prescribed GLP-1 drugs has increased between 2019 and 2024, starting at 0.19% to 1.33%. That's an increase of 587.8% and is likely due to the seemingly trending debate of "Wegovy vs Mounjaro" online.

But what do the stats say about those in the more senior age categories?

Although it is not something many people think about, GOV.UK figures show that as many as 70% of adults aged 65 and over are overweight or obese. Interestingly, a study from Yale (in the Journal of General Internal Medicine) researched whether polypharmacy can cause obesity in individuals aged 65 or older.

Due to this, as people enter their senior years, managing and maintaining a healthy weight can become increasingly important. Conditions such as arthritis and diabetes can make weight loss more difficult, while also limiting the ability to do the exercise that is often recommended to support it.

Weight loss medication use in the elderly does require more clinical intervention and a bit more TLC to be effective and safe. At the Bodyline Clinic, our complex care package includes those who are aged 70 or older, and, with 18 years of experience and a dedicated clinical team, we will not only help older people to lose weight safely, but we will help them to keep it off too.

Here, we will walk you through the whys and hows of this care package when you come to the Bodyline Clinic, in relation to oral and injectable GLP-1 medication Wegovy.

How Wegovy Works in Seniors

Very quick recap! Wegovy (both oral and injected) contains semaglutide. This is a GLP-1 receptor that has been approved for use in the UK to help adults lose weight. For more information on how to take Wegovy medications, check out our article on the Wegovy pill vs Wegovy injection here.

To qualify for this medication, adults need to have:

  1. A BMI of 30 or above
  2. A BMI of 27 or above with at least one weight-related condition like hypertension, type 2 diabetes, or high cholesterol

An important note: with older adults, BMI can become a less perfect measure. Muscle mass declines, and body composition changes. So, those who are seeking to offer complex care around GLP-1s for older adults, like the Bodyline Clinic, will usually assess waist circumference, mobility, frailty risk and strength rather than relying solely on the BMI. It is therefore advisable to use the NHS weight chart and consult a doctor for a more accurate BMI assessment than using the BMI online tool to start your weight loss journey.

Senior patients have an increased chance of developing adverse effects from any new medications that they take, including GLP-1s. They often have comorbidities, have age-related physiological complications, and they are also often taking multiple medications (polypharmacy).

As noted in our Wegovy Pills Side Effects Explained piece, common side effects of GLP-1s include:

  1. Nausea
  2. Vomiting
  3. Diarrhoea
  4. Constipation

These are common in all age groups and with both injectable and oral options.

But older adults who use this medication are at a higher risk of developing dehydration, electrolyte imbalance, reduced nutritional intake and acute kidney injury from taking this medication. Clinicians are also advised by Novo Nordisk to assess renal, kidney and hepatic (liver) function when starting and maintaining this medication in this age group and to adjust doses as needed. Those who are in this age group and taking this medication should also be given clinical guidance on how to use it safely.

Patients also need to be informed about the symptoms of potentially serious adverse effects and safety considerations and advised when to seek medical attention. When asking how long you take Wegovy for weight loss, this timeline may differ among age groups, with older people requiring a longer period to reach their weight loss goals.

Safety and Wegovy

There is a difference between monitoring safety when taking a new drug and monitoring effects.

It has been noted by clinical research that those taking GLP-1s later in life have a higher chance of developing secondary health complications, which are often urgent and require specialised intervention.

Gallbladder Disease

Semaglutide has been linked to issues with the gallbladder, such as cholelithiasis (gallstones) and cholecystitis (gallbladder inflammation).

If you experience persistent upper right abdominal pain, fever or jaundice, you should seek emergency medical attention.

Diabetic Retinopathy

If you have diabetes and are taking Wegovy, and if you have pre-existing diabetic retinopathy, you should be monitored. This medication may increase the risk of complications.

Hypoglycaemia

When you take Wegovy (or another GLP-1 designed for weight loss) and you have diabetes, the risk of hypoglycaemia increases. Why? When GLP-1s are mixed with insulin or sulfonylureas, the risk goes up.

So, talk to your doctor about dose reductions of those medicines if necessary.

Unsure about GLP-1s? Pros and Cons

You may be glancing back through this article and wondering what the upsides of this medication are if you are an older person.

There are many benefits of taking Wegovy later in life or other GLP-1 medications.

Potential Benefits

  1. You will lose weight, which can reduce stress on your joints, alleviate arthritis and improve your mobility.
  2. You may have improved metabolic health, such as lower blood pressure, lower cholesterol levels and better blood sugar control.
  3. Cardiovascular benefits! Semaglutide has been noted to reduce cardiac events by around 20% in older adults. So, that's great if you are an older adult who is already at risk of cardiovascular issues.

Potential Risks and Considerations

As before, you need to be aware of the potential complications that can come with taking Wegovy medication over the age of 75.

  1. Gastrointestinal side effects like nausea, diarrhoea, vomiting and constipation are common. These can lead to dehydration and electrolyte imbalances, which are more dangerous in older adults.
  2. Muscle and bone loss can occur. Rapid weight loss can reduce lean muscle and bone density. This increases the chance of both sarcopenia and fractures.
  3. Drug interactions can pose an issue. Older adults often take multiple medications (polypharmacy), and this can increase the chances of adverse issues.

FAQs

Can Wegovy be Prescribed to Patients Over 75 Years Old?

Yes, it can be prescribed to those over 75 years old. However, due to polypharmacy and potential health issues, this approach to medical weight loss needs to be tailored and monitored.

What Are the Main Safety Concerns When Prescribing Wegovy to Older Adults?

Key concerns that many pharmacies and online prescribing doctors have with prescribing any kind of weight loss medication to the elderly tend to relate to polypharmacy and potential health issues. Older patients also need to have their weight monitored more carefully, as well as their renal function and nutritional intake.

What Medical Checks Should Older Adults Have Before Starting Wegovy?

Before starting Wegovy, clinicians should assess:

  1. Your kidney function and liver function
  2. Any current medications
  3. Your blood pressure
  4. Your blood glucose (if appropriate)
  5. Mobility and frailty
  6. Nutritional status

These will help to assess whether these treatments will be suitable for you and will also help clinicians to see how closely you need to be monitored.
